HealtheLife Portal

DPH health department clients can use the DPH HealtheLife Portal to access their records online, such as immunizations, test results, schedule certain appointments and manage upcoming appointments. You can learn more about HealtheLife by reading frequently asked questions. 

How do I get access?

Request access during your next health department visit. You’ll receive your invitation by email to set up your username and password to access your health record. Once you have your username and password, download the app to start using HealtheLife.
