About Our Headquarters
As of June 2025, the South Carolina Department of Public Health’s (DPH) headquarters are co-located at the State of South Carolina Health Campus with the South Carolina Department of Behavioral Health and Developmental Disabilities, allowing for increased collaboration, coordination and communication between the two cabinet agencies. The state’s health campus is located at 400 Otarre Parkway in Cayce.
Services Not Affected by Headquarters Relocation
While the location of DPH’s headquarters has changed, some of the agency’s Columbia-based services will remain in place for now.
Vital Records
Birth and death certificates, marriage records or divorce reports will continue to be issued at DPH’s state Vital Records office, located at 2600 Bull St., Columbia, and at Vital Records offices across the state. These records are not available at the state’s health campus.
Richland County Health Department Services
Family planning, immunizations, STD/HIV/Hep C, Tuberculosis testing, Opioid OD Kits, and WIC services will continue to be provided at the Richland County Health Department, located at 2000 Hampton St., Columbia, and at public health clinics statewide.
Once Richland County completes its Family Services Center, the state’s Vital Records office and the Richland County Health Department will move to this new location at the former Columbia Place Mall. DPH will provide notice of these moves in advance.

What Visitors Need to Know
Tobacco-Free Campus
DPH has a Smoke and Tobacco-Free policy in place at South Carolina Health Campus and all other locations. The use of tobacco and smoking products — including dip/chew/oral nicotine pouches, e-cigarettes/vapes, and other tobacco products — is prohibited on DPH property. The policy applies 24 hours a day, seven days a week to all DPH employees, clients, contractors, vendors, and visitors (“covered individuals”) while on DPH property. In accordance with our commitment to promoting and protecting public health, all DPH offices and grounds are 100% smoke and tobacco-free campuses.
Campus Access
All visitors should enter the campus at the 12th Street Extension entrance. Visitors should enter through the far-left lane nearest the guard station and check in at the gate. The guard asks visitors for their identification, the purpose of the visit, the visiting agency and employee, and logs license plates.
Parking
Visitors may park almost anywhere on campus, however a spot close to the Building D entrance is recommended, if available. The parking garages have two levels with stair access; they do not have elevators. All handicap parking spaces are located on the top levels of the parking garages and in the surface parking lots. Visitors should not park on the loading dock or ramp at Building E, as this space is for delivery vehicles only.
Building Access
Visitors must use the entrance at Building D and check in with the main lobby guard, who will provide them with a visitor badge. The guard will contact the agency point of contact. The person the visitor is meeting with, or their designee, must meet them in the lobby and escort them to the meeting place. After the meeting, a DPH employee must escort the visitor back to Building D to return the visitor's badge and sign out.
Unexpected/unannounced visitors are expected to follow the same process and provide the same information.
