Transitioning to Adult Healthcare
DPH offers services to help youth, young adults, and their families plan for the future of their healthcare. These services teach young people how to take care of themselves and be part of their own medical decisions. They also help make the move from pediatric care to adult medical care smooth and organized.
Planning for the move to adult healthcare begins at age 12 for all Children and Youth with Special Health Care Needs (CYSHCN) clients. It can continue up to age 26 if needed. These services are offered to everyone, no matter their insurance or financial situation.
Support is provided at a level that matches each youth or young adult’s ability to be involved. The CYSHCN program is here to help you as you plan for what comes next.
Services provided for the move to adult healthcare for youth includes the following skills:
- assessing your child's ability to help manage their own health,
- making a doctor's appointment,
- filling a prescription,
- being aware of their personal and family health histories,
- asking questions during their visit,
- developing a medical summary and emergency care plan,
- discussing changes in privacy and consent that occur at age 18, and
- discussing their future goals.
Eligibility Requirements
- Enrolled in a Children and Youth with Special Health Care Needs (CYSHCN) program.
- Age: Adult healthcare transition services begin at 12 years old and may be extended up to 26 years old as needed.
