Mail-In/Online Only Requests for Marriage and Divorce

To get a certified copy of a marriage or divorce that occurred in South Carolina, submit the request by mail or online. In-person and drop-off services for these requests are temporarily unavailable. See mail-in and online directions for a Marriage Certificate or Divorce Report.

In-Person Amendment Requests at Regional Offices

Beginning November 17, 2025, the Richland County office (formally known as the State Office) will no longer take same-day amendment requests. All on-site amendment requests submitted to the regional offices, including the Richland county office, will be completed within 1-2 business days.

Section 44-63-140 of the South Carolina Code was amended effective May 16, 2023, to allow adoptees, age 18 or older, who were born in South Carolina, to obtain a copy of the adoptee's original birth certificate and any evidence of the adoption held with the original birth record, if the biological parent has completed the necessary forms consenting to the release of the certificate or is deceased. A biological parent may also indicate their contact preference and consent to release medical history information.

How Do I Change a Birth Certificate After an Adoption?

In order to amend a child's S.C. birth certificate following adoption, the attorney will usually submit the following:

  • Completed application form for the certified copy of a birth certificate
  • Certificate of Adoption (DPH form 0671 if adoption was ordered in South Carolina) or whichever form is used by the state in which the adoption occurred to report to vital records.
  • Certified copy of the Adoption Decree
  • Fees for the number of copies requested
  • $15 amendment fee

If you submit these items by mail, please be advised that processing generally takes 2 -3 months.

How Can I Start the Process for a Foreign Adoption?

The first actions occur at the Family Court. If you do not have an attorney, you can access the guidelines and forms at sccourts.org/forms. DPH does not issue any forms until the Family Court has completed their initial process.
