Our public health departments' hours of operation may be affected by power and staffing outages. This means that some closures may not appear on the state government office delays and closings map. The health and safety of our employees and our clients remains our highest priority. We are working to get services restored as safely and quickly as possible.

DPH’s CYSHCN program consists of a multidisciplinary team to meet your specific needs. Our care coordinators consist of Registered Nurses, Registered Dietitians, and Licensed Social Workers who are all adept at navigating the health care system and providing additional resources for you and your family.

Our team focuses on care coordination to ensure appointments are kept, referrals are initiated, needs, priorities, or concerns are addressed, and transition planning for transitioning to adult health care systems.

SC DPH also partners with Family Connection of SC, our South Carolina Parent-to-Parent Organization, that can provide additional support and resources and support to you and your family.
