Renewal Application

DPH does not have an online renewal process. Renewal applications must be sent through U.S. postal mail to the address on the application.

If you are attempting to renew an "active" (not expired) S.C. Controlled Substances Registration, use the renewal application:

If you have not received a renewal application, you may print a duplicate:

What if My Registration Expires?

The registration of a registrant who fails to renew by April 1 or October 1, whichever is applicable, is expired. On May 18, 2018, the South Carolina Controlled Substances Act eliminated the registration grace period. See the South Carolina Code of Laws Section 44-53-280 (C & D) for details.

To reinstate an expired registration, a new application must be submitted with a payment of the registration fee plus a $100 penalty fee. All applications must be submitted through U.S. postal mail to the Bureau of Drug Control. Processing time is approximately 12-15 business days. Visit DPH's New Registrations webpage for more information.

Providers are not authorized to prescribe, administer, possess or dispense controlled substances until their S.C. Controlled Substances Registration is reinstated.

Check on the Status of a Renewed Controlled Substance Registration

Check on the status of your renewal:

If you are (1) unable to locate your registration or if you have (2) submitted the renewal application and paid the fees but see an expired registration, contact the Bureau of Drug Control at bdcrenewals@dph.sc.gov or 803-896-0636 for assistance.
