Please complete the following form for product specification and submission.

Thank you for providing product name(s) UPC(s) and package images for South Carolina WIC-Approved Products.

This form MUST BE USED FOR ALL FOOD PRODUCT SUBMISSION.
No other forms will be accepted (e.g disc or thumb drive).

Prior to submitting, refer to the SC WIC Approved Foods Nutrition Criteria prior to submitting any food product.
