Please review BCN's eligibility requirements to confirm you meet all qualifications. If eligible, you may contact one of the providers listed below and request an appointment as a new patient under BCN coverage. Each site is an active enrollment provider that is authorized to offer services under BCN. These providers will manage your authorization for coverage which will be valid for one year (for breast and cervical services only). Enrollment is completed by provider staff. If you visit a provider that is not on this list, we cannot guarantee coverage.
There are no costs to you for utilizing BCN coverage for breast and cervical procedures. BCN covers a wide range of services, from screenings to diagnostic procedures. When calling to schedule your appointment, please ask about any required documents, which may include a form of ID, proof of residency, proof of income, and proof of insurance.
Your appointment will be an annual women’s wellness visit, during which you will complete some brief paperwork. The visit may include a clinical breast exam, pap test, and/or HPV test. You will also have the opportunity to discuss referrals for additional services, including mammograms and follow-up diagnostic care. The enrollment site will issue an order to cover these services under our funding.
