SIMON is an immunization information system that helps ensure access and availability of essential immunization records. It also allows DPH to assess immunization coverage rates across the state. DPH launched SIMON in September of 2020 with funding from private and public grants.
This portal is like the online patient health record system most hospitals and primary care physicians use to make other health records available online to patients. Using the SIMON Public Portal, South Carolinians have historically been able to view and download their personal vaccination records from home.
Beginning Aug. 1, 2025, parents will be able to use the portal to download the South Carolina Certificate of Immunization for their school-aged child who is compliant with the current South Carolina Immunization Requirements, and share it with their child’s school.
There are two immunization records available in the SIMON Public Portal:
- The South Carolina Certificate of Immunization (DPH form 4024) is the only record accepted for attendance to school and childcare.
- The South Carolina Personal Immunization Record (DPH form 4025) is a record of all the immunizations given to someone throughout their lifetime. This record is not valid for use as the Immunization Certificate for attendance at school and childcare.
Yes. Parents can easily access the personal immunization record of their minor child and print it out or save it to their computer.
However, it is important to note that while the printed record helps parents know where their child stands in terms of vaccinations, at this time, it cannot be used to replace the immunization certificate that must be signed by a provider to verify immunization status for entrance to school and/or childcare. DPH is working to make that an option.
The system is easy to use. Users provide basic information of the person whose record is needed (first and last name, gender, date of birth), along with a phone number or email that has already been entered in SIMON by their provider. If you are unable to obtain your immunization record due to missing information (first and last name, gender, date of birth, phone number, and email), you can get it by making a SIMON Vaccination Record Request. Once you complete the form, our SIMON team will contact you via email informing you that you may now access you or your dependent’s immunization record.
Yes. The information provided about the person whose record is needed is used to develop a two-factor authentication to securely access immunization records:
- Demographic information must match a record in SIMON, including an email address.
- A verification code will be sent via email. The code must be entered in the public portal to gain access to either the school certificate of immunization or personal immunization record.
South Carolina is among many states and other entities that have systems that allow members of the public to access their immunization records. 85% percent of jurisdictions that responded to a survey by the Association of Immunization Managers said they already are offering public access to their immunization records or plan to implement or enhance the service in the coming year.
Vaccinations administered in some other states or in military facilities are not required to be reported in South Carolina. Records of these immunizations may not be found in the public portal.
For military families looking to gain access to their vaccination records, we advise you to speak with the clinic associated with your base. SIMON does not currently receive information from the military. However, the Military Certification of South Carolina School and Childcare Immunization Requirements (DPH form 4375) is available to use in conjunction with the child’s vaccination record DD-2766-C or P-0630 form.
As of Jan. 1, 2017, all immunization providers are to report all administered doses of vaccines to the state immunization registry. If you have questions regarding your immunizations, please reach out to your primary health care provider or the provider that would have administered the vaccines.
