Our public health departments' hours of operation may be affected by power and staffing outages. This means that some closures may not appear on the state government office delays and closings map. The health and safety of our employees and our clients remains our highest priority. Annual Inventory

The annual controlled substance inventory is due on May 1 of each year , unless another date is approved in writing by this office.

Registrants must be certain to record the date of the inventory and whether the inventory was taken at the opening or the close of the business day, and the time the inventory is taken. Such inventory shall be signed by a responsible individual, who attests to the completeness and accuracy of the inventory. Inventory records must be maintained for two years in a readily retrievable manner.

Providers and facilities that missed their renewal deadline will not be able to renew by submitting their payment through the online payment portal on this website. Payments submitted via the online payment portal will be credited back to the credit card account. Checks and/or money orders will be returned.
