Community Health Workers Connect People to Resources

- Accessing food, clothing, or other daily essentials
- Completing WIC, SNAP, or Medicaid applications
- Finding a doctor or healthcare provider
- Getting financial assistance
- Meeting other needs such as transportation, job seeking, or similar support
The South Carolina Department of Public Health can help. Community Health Workers (CHWs) are trained staff members who are trusted in their communities and understand the people they serve. They can help connect you with the resources you need. Once you complete the form below, a CHW will follow up to discuss your needs:

Recent Ways CHWs have supported South Carolinians
- Connecting a pregnant woman with prenatal care and WIC
- Helping a non-English speaker navigate the hospital system after a cancer diagnosis
- Securing free car seats for a mother with limited financial resources
- Using community connections to obtain a wheelchair for someone in need
- Supporting an individual in earning a GED to improve employment opportunities
CHW Key Roles and Responsibilities
Empower
CHWs help community members identify and address barriers to health—such as transportation challenges, limited access to care, or lack of nutritious food—to improve overall well-being.
Coordinate Care
CHWs help individuals apply for and enroll in services like WIC, SNAP, and Medicaid, which support nutrition, healthcare access, and overall health.
Partner to Provide Health Services
CHWs partner with communities to coordinate mobile vaccination and testing clinics that reduce the risk of vaccine-preventable diseases.
Provide Information & Education
CHWs provide health information through one-on-one coaching, community presentations, workshops, and educational outreach at local events.
