Live Healthy South Carolina, a statewide effort led by the Department of Public Health (DPH) in partnership with the Alliance for a Healthier South Carolina and multi-sector partners from across the state, is pleased to present South Carolina’s 2025-2030 State Health Improvement Plan (SHIP).

The SHIP serves as a statewide strategic roadmap to improve population health and address South Carolina’s most pressing health challenges. It reflects intentional and robust collaboration among state agencies, public health entities, health care providers, community-based organizations, and other key partners to develop and implement sustainable, data-driven solutions that achieve collective impact. The SHIP contains nine priority areas that were developed using analytic insights from the comprehensive health data in the State Health Assessment (SHA) and cross-sector partner input. The SHIP provides a strategic framework for multisector partners to align resources and guide collective action to improve health outcomes across South Carolina.

State Health Assessment (SHA)
South Carolina’s 2023 State Health Assessment (SHA) is a comprehensive evaluation of the health status of South Carolinians designed to inform health improvement plans at the state and community levels. In addition, it serves as a health data resource that organizations, the media and the public can use.
Live Healthy South Carolina, a statewide effort between the South Carolina Department of Public Health (DPH) and the Alliance for a Healthier South Carolina (Alliance), is pleased to present South Carolina’s 2023 State Health Assessment (SHA), a comprehensive resource of the latest public health data, issues and trends affecting all South Carolinians.
The SHA is a critical tool for population health agencies and programs, health care practitioners, policymakers, researchers, communities, and other professionals to examine key health indicators, identify health disparities and monitor trends and progress in the field of public health. It builds on extensive data analysis and community engagement and highlights capacity throughout our state, including resources and assets that work collaboratively to improve the health of South Carolina residents.
